New research expands the number of women with a common type of early stage breast cancer who can skip chemotherapy and safely be treated with hormone-blocking therapies alone – even if their cancer has spread to some of their lymph nodes.[1] However, women whose tumors have spread to their lymph nodes are often at higher risk of worse outcomes, with the majority receiving chemotherapy.[2]
Results from the RxPONDER study, one of the largest clinical trials in hormone receptor-positive, HER2 negative early breast cancer, were presented at the San Antonio Breast Cancer Symposium in late 2020.1<br />
Based on these new findings, postmenopausal women whose cancer has spread to between one and three lymph nodes can avoid the side effects associated with chemotherapy if their Oncotype DX Breast Recurrence Score® result, a genomic test which predicts chemotherapy benefit and risk of cancer recurrence, is between 0 and 25.1<br />
Breast cancer is the most commonly diagnosed cancer worldwide, surpassing lung cancer for the first time in 2020.[3] With the growing recognition of the importance of personalized treatment for breast cancer, genomic testing can help tailor treatment decisions to a patient’s individual tumor. Facing a breast cancer diagnosis can be overwhelming but getting a genomic test to understand the unique biology of a patient’s tumor can help guide personalized treatment decisions.<br />
Our guest, Dr. Deepa Halaharvi, a board-certified breast cancer surgeon at Ohio Health, can speak to the practice-changing impact of this recent study for postmenopausal women with node-positive breast cancer and the importance of genomic tests in enabling confident breast cancer treatment decisions.<br />
As a breast cancer survivor herself, Dr. Halaharvi is in the unique position of seeing breast cancer from the perspective of a doctor as well as a patient. She will share her cancer story, including how the Oncotype DX® test informed her treatment, following her diagnosis with stage 1 HR positive, HER2 negative breast cancer at age 42, just 8 months after becoming a practicing breast surgeon. <p>I’m following up my email last week, after Iranian President Hassan Rouhani declared over 25 million Iranians have contracted COVID, to bring your attention to a U.S. based organization aiding relief efforts both here and in Iran. I’m hoping you’ll take interest in this story as a standalone focus, orin tapping into the organization’s leadership for commentary on this or other related affairs. </p>
<p>Moms Against Poverty (MAP) is a global non-profit organization, founded in 2008 in San Francisco as a volunteer-based nonprofit organization to nurture and educate underprivileged children. In 2020, MAP is working diligently to aid children and the poor impacted by COVID-19. They are active in helping U.S. families,having provided over 50,000 meals in 2020 alone for families from San Francisco to Washington, D.C. but are also currently one of the only U.S. based charities able to provide aid to Iran. </p>
<p>In regards to Iranian humanitarian efforts, the decades-long political stalemate, and severe sanctions / banking restrictions imposed by the U.S., inhibit availability of medical equipment to stem the coronavirus’ effects. Support is desperately needed for the most vulnerable of the populace. </p>
<p>This year alone, MAP has supported over 200,000 children and their families in Iran by providing 27 ventilators, 7 ECGs, 6 radiography machines, 3 oxygen machines, 150 oxygen tanks and tens of thousands of various forms of critical PPEs, including isolation clothing, surgical gowns, masks,and face shields. Beyond hospital support, MAP also donated PPEs, disinfectants and surface cleaners to public orphanages and homeless shelters in Iran. </p>
<p>While this is an accomplishment that tackles the first phase of the pandemic, to alleviate the ever-increasing economic impact of COVID-19, MAP has transitioned their effort to hunger relief in Iran. They recently distributed food packets including groceries such as meat, rice and more to 2000 families. Additionally,MAP has launched a bread project where families in Iran receive bread for a month. MAP also have had a disaster relief NGO arm, operating in Iran since 2010. Recently their funds ceiling quota was tripled by OFAC, allowing them to do even more work. More information on the organization’s efforts is in their mid-year report, here. </p>
<p>While there is brinksmanship ongoing between the U.S. and Iran, the suffering of innocents due to COVID is something that should be addressed and helped at all costs. </p>

Lemons (and limes) were very valuable at the time for the protection they offered against scurvy. During the Gold Rush in California, lemons were so popular and in such unbelievably high demand that people were willing to pay the unheard price of $1 each. This would be expensive even for today, but imagine that in the 1800’s!</p>

Until December 2016, there were no treatment options available for the nearly 9,000 individuals in the United States living with a rare disease called spinal muscular atrophy (SMA).  Individuals with SMA often experienced symptoms related to the deteriorating effects of the condition, which may include the gradual onset of muscle weakness that can lead to the inability to stand or walk independently.<br />
With scientific and treatment advances, patients of all ages have now been given hope. While commonly thought of as a pediatric disease, today 65% of individuals living with SMA are adults. One adult living with SMA is 28-year-old Kailey McCallister.<br />
Despite being diagnosed with SMA at an early age, Kailey refused to allow the disease to keep her from having a positive outlook on her life and is eager to share her story. Kailey is joined by Dr. Margaret Frey, a neurologist specializing in SMA at Memorial Healthcare in Michigan, discusses the medical advances and initiatives that have helped the community fight back against this progressive disease<br />

The term <i>palsy</i> in modern parlance refers to disorder of movement, but the word root &#8220;palsy&#8221; does still technically mean &#8220;<a title="Paralysis" href="http://en.wikipedia.org/wiki/Paralysis">paralysis</a>&#8221; today, even though it is not used as such within the meaning of cerebral palsy. The use of &#8220;palsy&#8221; in the term cerebral palsy makes it important to note that <a title="Paralysis" href="http://en.wikipedia.org/wiki/Paralysis">paralytic disorders</a> are in fact <i>not</i> cerebral palsy – meaning that the condition of <a title="Quadriplegia" href="http://en.wikipedia.org/wiki/Quadriplegia">quadriplegia</a>, which comes from <a title="Spinal cord injury" href="http://en.wikipedia.org/wiki/Spinal_cord_injury">spinal cord injury</a> or <a title="Traumatic brain injury" href="http://en.wikipedia.org/wiki/Traumatic_brain_injury">traumatic brain injury</a>, should not be confused with <a title="Spastic quadriplegia" href="http://en.wikipedia.org/wiki/Spastic_quadriplegia">spastic quadriplegia</a>, which doesn&#8217;t; nor should <a title="Tardive dyskinesia" href="http://en.wikipedia.org/wiki/Tardive_dyskinesia">tardive dyskinesia</a> be confused with <a title="Dyskinetic cerebral palsy" href="http://en.wikipedia.org/wiki/Dyskinetic_cerebral_palsy#Athetoid.2FDyskinetic">dyskinetic cerebral palsy</a>, or the condition of (paralytic) &#8220;<a title="Diplegia" href="http://en.wikipedia.org/wiki/Diplegia">diplegia</a>&#8221; with <a title="Spastic diplegia" href="http://en.wikipedia.org/wiki/Spastic_diplegia">spastic diplegia</a>. In fact, as of the early 21st century some clinicians have become so distressed at common incorrect use of these terms that they have resorted to new naming schemes rather than trying to reclaim the classic ones; one such example of this evolution is the increasing use of the term <i>bilateral spasticity</i> to refer to <i>spastic diplegia</i>. Such clinicians even argue quite often that the &#8220;new&#8221; term is technically more clinically accurate than the established term.</p>
<p>Cerebral palsy&#8217;s nature as a broad category means it is defined mostly via several different subtypes, especially <a title="Spastic cerebral palsy" href="http://en.wikipedia.org/wiki/Spastic_cerebral_palsy">the type featuring spasticity</a>, and also mixtures of those subtypes.</p>
<p>Cerebral palsy is caused by damage to the motor control centres of the developing brain and can occur during <a title="Pregnancy" href="http://en.wikipedia.org/wiki/Pregnancy">pregnancy</a>, during childbirth or after birth up to about age three. Resulting limits in movement and posture cause activity limitation and are often accompanied by disturbances of sensation, depth perception, and other sight-based perceptual problems, communication ability; impairments can also be found in <a title="Cognition" href="http://en.wikipedia.org/wiki/Cognition">cognition</a>, and <a title="Epilepsy" href="http://en.wikipedia.org/wiki/Epilepsy">epilepsy</a> is found in about one-third of cases. CP, no matter what the type, is often accompanied by secondary musculoskeletal problems that arise as a result of the underlying disorder. Improvements in <a title="Neonatology" href="http://en.wikipedia.org/wiki/Neonatology">neonatology</a> (specialized medical treatment of newborn babies) have helped reduce the number of babies who develop cerebral palsy and increased the survival of babies with very low birth weights (babies which are more likely to have cerebral palsy). A 2007 six-country survey found an incidence of CP of 2.12–2.45 per 1,000 live births, indicating a slight rise in recent years. A 2003 study put the average lifetime cost for people with CP in the US at $921,000 per individual, including lost income.</p>
<p>Of the many types and subtypes of CP, none has a known cure. Usually, medical intervention is limited to the treatment and prevention of complications arising from CP&#8217;s effects.</p>
