<p><b>Stephanie, Obstructive HCM Patient<u></u><u></u></b></p>
<p>Stephanie is a trained occupational therapist located in Natchez, Mississippi. In 2022, Stephanie was diagnosed with symptomatic obstructive hypertrophic cardiomyopathy, and since then has become passionate about raising awareness of the symptoms and impact of the disease. She is now a part-time yoga teacher who enjoys volunteering, pottery, hiking, walking and spending time with her husband and three sons.<u></u><u></u></p>
<p><b>William H. Crowder, MD, board-certified cardiologist, Jackson Heart<u></u><u></u></b></p>
<p>Born and raised in Grenada, MS, Dr. Crowder attended Millsaps College where he graduated magna cum laude with a degree in biology. He attended medical school at the University of Mississippi, pursuing training in internal medicine and cardiology. Finally, Dr. Crowder underwent a year of additional training in advanced interventional cardiology techniques at the Medical University of South Carolina. There he trained with national leaders in the interventional cardiology community with an emphasis on atrial septal defect/patent foramen ovale repair as well as techniques for treatment of hypertrophic cardiomyopathy.<u></u><u></u></p>
<p>He is active in multi-center research trials, staying at the forefront of medical technology.<u></u><u></u></p>
<p>Dr. Crowder is board certified in internal medicine, cardiovascular disease, and interventional cardiology. His memberships include the Mississippi State Medical Association, the American Medical Association, and the American College of Cardiology. He is a fellow of the College of Cardiology. He has been active in the medical community and is a past member of the board of trustees of the state medical association.<u></u><u></u></p>
<p>He attends St. Richard’s Church with his wife, Kimberly. When he is not working, he enjoys reading and playing music. He is an accomplished pianist and works with his father on various record projects.<u></u><u></u></p>

<p>CAMZYOS® (mavacamten) is a prescription medicine used to treat adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM). CAMZYOS may improve your symptoms and your ability to be active. It is not known if CAMZYOS is safe and effective in children.<u></u><u></u></p>
<h1>IMPORTANT SAFETY INFORMATION<u></u><u></u></h1>
<p><b>CAMZYOS (mavacamten) may cause serious side effects, including:<u></u><u></u></b></p>
<p><b>Heart failure, a condition where the heart cannot pump with enough force. </b>Heart failure is a serious condition that can lead to death. You must have echocardiograms before you take your first dose and during your treatment with CAMZYOS to help your healthcare provider understand how your heart is responding to CAMZYOS. People who develop a serious infection or irregular heartbeat have a greater risk of heart failure during treatment with CAMZYOS. Tell your healthcare provider or get medical help right away if you develop new or worsening shortness of breath, chest pain, fatigue, swelling in your legs, a racing sensation in your heart (palpitations), or rapid weight gain.<u></u><u></u></p>
<p><b>The risk of heart failure is also increased when CAMZYOS is taken with certain other medications. </b>Tell your healthcare provider about the medicines you take, both prescribed and obtained over-the-counter, before and during treatment with CAMZYOS.<u></u><u></u></p>
<h1>Because of the serious risk of heart failure, CAMZYOS is only available through a restricted program called the CAMZYOS Risk Evaluation and Mitigation Strategy (REMS) Program.<u></u><u></u></h1>

As the pandemic continues, reducing the risk of heart failure hospitalization is more important than ever</p>
<p>It’s always critical for people with heart failure (HF) to take steps to keep healthy and to try to reduce the risk of hospital visits –  but now, as these patients face increased health risks and hospitals are overburdened, it’s more important than ever.</p>
<p>HF is a chronic, progressive condition that can change the structure of the heart so it is unable to pump enough blood to the body. Patients with HF are inherently at increased risk of hospitalization for symptoms like shortness of breath or swelling – often in the legs, ankles, feet and/or abdomen. Hospitalizations for HF are a hallmark of disease progression, or worsening of the condition.</p>
<p>Annually, HF accounts for about 1 million hospitalizations. That’s about 2 HF hospitalizations every minute in the U.S.</p>
<p>A visit to hospital for heart failure can average between five and ten days – further, around one in four patients are re-admitted within 30 days of discharge, and one in ten may not survive.</p>
<p>These recurrent experiences can profoundly disrupt the lives of patients and caregivers.</p>
<p>For people living with heart failure, hospitalizations can be a recurring part of life. But finding the right care plan with a health care provider can help people actively manage their condition to reduce their risk of HF hospitalizations. </p>
<p>Kim, for example, is a patient who used to experience frequent HF hospitalizations before changing her treatment regimen. Looking back, this hardworking single mom was referred to a lung specialist by her primary care physician because of a persistent cough that wouldn’t go away. At just 32 years of age, she was shocked to be diagnosed with chronic heart failure, a condition that would lead her to endure frequent heart failure hospitalizations over the next several years.</p>
<p>Fortunately, Kim was able to work with her health care team on a plan, including a healthy lifestyle and treatment regimen that would help manage this chronic condition. In fact, there are steps HF patients can take to help reduce the risk of a HF hospitalization. These can include following a low-salt diet, getting an appropriate amount of physical activity, and maintaining the treatment regimen advised by their health care provider.</p>
<p>Beth Davidson, a heart failure nurse practitioner in Nashville, TN, and President of the American Association of HF Nurses, will be available to discuss the prevalence of HF, a condition affecting over 6 million people in the U.S., and projected to affect approximately 8 million by 2030. She can discuss the seriousness of HF hospitalizations, HF symptoms, treatment and lifestyle tips that may help patients or a loved one manage the condition. Kim will also share her story and how she is successfully managing her HF today.</p>

<p>Dr. Mikhail Kosiborod is a cardiologist, the Vice President of Research at Saint Luke’s Health System, Director of Cardiometabolic Research and Co-Director of the Haverty Cardiometabolic Center of Excellence at Saint Luke’s Mid America Heart Institute, and Professor of Medicine at the University of Missouri-Kansas City. Dr. Kosiborod received training in clinical research, epidemiology and health policy through the Robert Wood Johnson Clinical Scholars Program, as well as clinical training in cardiovascular medicine at Yale University School of Medicine. Dr. Kosiborod is an internationally recognized expert in the fields of diabetes and cardiovascular disease, cardiometabolic and cardiorenal syndromes, as well as quality of care and outcomes. Dr. Kosiborod has authored and co-authored over 220 peer-reviewed publications, including scientific statements and position documents. Dr. Kosiborod is involved in the leadership of multiple clinical trials and multi-center registries, and is currently the principal investigator of several investigator-initiated, multicenter trials in diabetes and cardiovascular disease.</p>

<p>We had no childproof lids on medicine bottles, locks on doors or cabinets,<br />
And, when we rode our bikes,<br />
We had baseball caps,<br />
Not helmets, on our heads.    </p>
<p>As infants and children, we would ride in cars with no car seats, no booster seats, no seat belts, no air bags, bald tires and sometimes no brakes..    </p>
<p>Riding in the back of a pick- up truck on a warm day was always a special treat.     </p>
<p>We drank water from the garden hose and not from a bottle.    </p>
<p>We shared one soft drink with four friends, from one bottle, and no one actually died from this.     </p>
<p>We ate cupcakes, white bread, real butter, and bacon. We drank Kool-Aid made with real white sugar.<br />
And we weren&#8217;t overweight.<br />
WHY?<br />
Because we were always outside playing&#8230;that&#8217;s why!     </p>
<p>We would leave home in the morning and play all day, as long as we were back when the streetlights came on.</p>
<p>No one was able to reach us all day   .. .<br />
And, we were OKAY.    </p>
<p>We would spend hours building<br />
Our go-carts out of    scrapsand<br />
then ride them down the hill,</p>
<p>Only to find out that we forgot about brakes. After running into the bushes a few times, we learned  To Solve the problem.    </p>
<p>We did not<br />
Have Play Stations, Nintendo<br />
and X-boxes. There were<br />
No video games,<br />
No 150 channels on cable,<br />
No video movies<br />
Or DVDs,<br />
No surround-sound or   CDs,<br />
No cell phones,<br />
No personal computers,<br />
No Internet and<br />
No chat rooms.<br />
WE HAD FRIENDS<br />
And we went<br />
Outside and found them!      </p>
<p>We fell out of<br />
trees, got cut,<br />
Broke bones and<br />
Lost teeth,<br />
And there were<br />
No lawsuits<br />
From those accidents.    </p>
<p>          We would get<br />
Spankings with wooden spoons, switches, ping-pong paddles, or just a bare hand,<br />
And no one would call child services to report abuse.     </p>
<p>           We ate worms,<br />
And mud pies<br />
Made from dirt,<br />
And<br />
The worms did<br />
Not live in us forever.      </p>
<p>We were given<br />
BB guns for our 10th birthdays,<br />
22 rifles for our 12th, rode horses,<br />
made up games with sticks and<br />
tennis balls, and  </p>
<p>   -although we were<br />
Told it would happen- we did not put out very many eyes.     </p>
<p>We rode bikes<br />
Or walked to a friend&#8217;s house and knocked on the door or rang the bell,<br />
or just Walked in and talked to them.</p>
<p>Little League had<br />
tryouts<br />
And not everyone<br />
Made the team.<br />
Those who didn&#8217;t<br />
Had to learn<br />
To deal with<br />
Disappointment.      </p>
<p>Imagine that!!    </p>
<p>The idea of a parent<br />
Bailing us out<br />
If we broke the law<br />
was unheard of &#8230;<br />
They actually sided with the law! </p>
<p>These generations have<br />
Produced some of the best risk-takers,<br />
Problem solvers, and<br />
Inventors ever.      </p>
<p>The past 60 to 85 years<br />
Have seen an explosion<br />
of innovation and new ideas.        </p>
<p>We had freedom,<br />
Failure, success and responsibility,<br />
and we learned</p>
<p>Ho w to deal with it all.         </p>
<p>       If YOU are    One of those born<br />
Between 1925-1955, CONGRATULATIONS!     </p>
<p>     You might want<br />
to share this with others who have had the luck to grow up as kids before the lawyers<br />

<p style="color: #000000;">A Dot Earth reader, Jay Rappoport, followed up on Peter Barnes’s proposal for a “cap and dividend” system with another take on the idea that’s worth highlighting here, if only because the central metaphor is so easy to absorb — having a “bottle bill” for climate.</p>
<p style="color: #000000;">Of course, one problem with most bottle bills around the United States is the fee hasn’t risen to adjust for inflation, so many bottles still go into the trash because the five-cent surcharge is a yawn. And many drink containers (like all those water bottles!) are left out in most cases. (This recent Green Counsel post about troubles with an updated bottle-bill law in New York State is worth a look.)</p>
<p style="color: #000000;">In the case of climate, the fee would have to steadily rise to propel a steady shift away from energy choices that come with a lot of carbon dioxide releases, and it’d have to encompass all sources (and perhaps credit “sinks,” like new forests, that sop up the gases). Is that sellable?</p>
<p style="color: #000000;"><span style="text-decoration: underline;">Here’s Mr. Rappoport’s thesis</span>:</p>
<p style="color: #000000;">I have been nursing a similar concept for perhaps 25 years, which I had first conceived in a sophomore economics class back in College in the early 80’s, when the ideas of externalities and market failure came together for me.</p>
<p style="color: #000000;">I believe this principal is as important and fundamental to understanding the general progress of human civilization, as the concept of erosion is to geology.</p>
<p style="color: #000000;"><span style="text-decoration: underline;">Without further delay, here it is</span>:</p>

<p style="color: #000000;"><span style="text-decoration: underline;"><strong><em>Introduction</em></strong></span></p>
<p style="color: #000000;">The question to be addressed here is that of managing human generated global warming specifically, and more generally the larger question of sustainable growth, as all follow from the same underlying causes, and are amenable of the same solution. What follows is a brief discussion of the issues, and an effort to reframe, refine and build upon the emerging consensus view concerning the ‘cap and trade’ approach, and to suggest the minimum regulatory structure necessary to this end. The structure suggested here may be public, private, or collaborative.</p>
<p style="color: #000000;"><span style="text-decoration: underline;"><em><strong>Economic vs. Natural property</strong></em></span></p>
<p style="color: #000000;">The fundamental underlying issue in this discussion is property rights. Everything that follows, turns on understanding the role of property rights in the market.</p>
<p style="color: #000000;">So to begin, consider everything in the world as a ‘good’, and falling into one of two types; as either part of ‘The Economy’ and therefore an ‘economic good’, or alternatively, ‘The Natural Environment’ and as such, an ‘environmental good’. The only difference being, that ‘goods’ in the economy have a defined property right, and that ‘goods’ in the environment do not. (Often referred to as “externalities” in economics, examples of environmental goods might be the polar ice caps, unpolluted water, the earth’s atmosphere, and so on).</p>
<p style="color: #000000;">An economic good can be bought and sold, only because it is owned by someone who had the property right to sell it. An environmental good cannot be sold, because no one owns it to sell it. As an example, imagine a stranger offering to “sell you The Brooklyn Bridge”.</p>
<p style="color: #000000;"> In order to sell an environmental good, ownership must first be established, at which point it becomes an economic good, and can then be sold. So, until ownership has been established, an environmental good cannot be sold, nor hold a price in the market.</p>
<p style="color: #000000;"><em><span style="text-decoration: underline;"><strong>Markets, Pricing &amp; Incentives</strong></span></em></p>
<p style="color: #000000;">Because environmental goods lack a property right and are therefore unable to hold a price in the marketplace, so far as the market is concerned, their ‘pricing’ is effectively pegged at zero value. This perception of the pricing of environmental goods as being at or nearly zero has profound consequences in the economic arena.</p>

(Consider for example the proliferation of spam email, with a marginal cost of zero per email. How much spam would be eliminated if there were a cost of 0.001 cent per email?)</p>
