<p>HCP Bio: Dr. James Dugan</p>
<p>Dr. James Dugan is a hematology/oncology specialist, and the physician lead for the transplant and cellular therapy department for Novant Health Cancer Institute in Winston-Salem, North Carolina.</p>
<p>Dr. Dugan was drawn to the field of oncology by the opportunity to improve a person&#8217;s life during a difficult time. He believes that to properly treat the whole person and not just the disease, physicians need to engage in active conversations with the patient throughout the treatment process. Dr. Dugan approaches medicine and oncology as a partnership between the physician and the patient.</p>
<p>Dr. Dugan is a member of the American Society of Hematology, as well as the American Society for Transplant and Cellular therapy. His membership in these organizations keeps him up-to-date on the latest research and treatment options in the field.</p>
<p>When he is not seeing patients, Dr. Dugan enjoys time with his wife and three daughters. He is an outdoor enthusiast who enjoys skiing, scuba diving, kite surfing, and long-distance running. He is passionate about the shared human experience and loves meeting new people and exploring new ideas and places.</p>
<p>Patient Bio: Jen</p>
<p>In 2014, Jen was diagnosed with Philadelphia chromosome positive chronic myeloid leukemia (Ph+ CML) in chronic phase (CP).<br />
Prior to her diagnosis, Jen devoted her career to serving others. After dedicating 20 years to running inner-city youth programs, Jen became a social worker, supporting women as they rebuilt their lives after incarceration or experiencing domestic violence.<br />
Jen was actively working as a social worker when she was diagnosed with CML. Unfortunately, her diagnosis required her to step away from her career for a period of time. Working closely with her doctor, she navigated the treatment landscape to find a treatment option that worked for her.<br />
Today, she is a teacher in her rural community, educating K-8 students in a one-room schoolhouse. She is also passionate about sharing her journey and advocating for other adults with CML, helping them access the information and support they need to have meaningful conversations with their doctors and navigate their own patient journeys.</p>

<p>A diagnosis of chronic lymphocytic (LIM-foh-SIH-tik) leukemia (C-L-L) can be overwhelming, but advances in cancer research, including targeted therapies, have paved the way for more precise treatments. C-L-L is one of the most common blood cancers in adults, and because its progression varies from person to person, choosing a tailored treatment plan is crucial.</p>

<p>Today, we’re joined by Lynne who will share her personal journey with C-L-L, and Dr. Nakhle Saba [Knack-Leigh Sah-bah], director of lymphoid malignancy and CAR T-cell (Car-tee cell )therapy at Our Lady of the Lake Cancer Institute, who will provide insight into the latest treatment approaches and why informed decision-making is so essential for those facing this disease.</p>

<p>Dr. Saba earned his medical degree from the Lebanese University in Beirut 2005. He completed his residency in Internal Medicine at East Tennessee State University in Johnson City, TN and Tulane University in New Orleans, LA, and his fellowship in Hematology and Medical Oncology at Tulane University. After fellowship completion in 2011, Dr. Saba joined the National Heart, Lung, and Blood Institute (NHLBI), National Institutes of Health (NIH) for an additional fellowship in lymphoid malignancies. His research at the NIH was focused on tumor biology and developing new therapeutic approaches for leukemia and lymphoma. He returned to Tulane in 2014 as Assistant Professor of Medicine, promoted to Associate Professor in 2018, then to Professor in 2024 following a successful career in research, teaching, and treating blood cancers. Dr. Saba’s research focuses on lymphoid malignancies and cell therapy, to identify therapeutic targets and novel drugs for these diseases.</p>
<p>Dr. Saba’s leadership skills are evident by the roles he played at Tulane as chair of the “immune effector cell therapy – IECT”, co-chair of Sarah-Cannon Lymphoma Committee, and President of Louisiana Oncology Society (LOS). Under his direction as president, the LOS passed four bills that were signed into law in Louisiana and are expected to positively impact cancer care in our state. Those bills include the Bone Marrow Donation, Fertility Preservation For Cancer Patients, Right To Prompt Coverage, and Expansion Of Biomarker Testing.</p>
<p>Dr. Saba authored and/or co-authored more than 120 published manuscripts and research abstracts, he presented at regional, national, and international meetings as a researcher and invited speaker. His articles appear in journals like Blood, Leukemia, Cancer Research, The Lancet Haematology, Haematologica, Science Advances, Bone Marrow Transplant and others. <p>Dr. Saba was recruited at the Our Lady of the Lake Cancer Institute as Professor of Medicine at LSU Health Sciences Center, and Director of Lymphoma, CLL, and chimeric antigen receptor (CAR)-T. He will be diligently working to expand the lymphoma and CLL research portfolio and bring cutting edge therapies to the region such as CAR-T, the next generation CARs, and be the first to bring the tumor infiltrating lymphocytes (TILs) for solid tumors to Louisiana.</p>
<p>About Lynne</p>
<p>I am a very active, “young” senior citizen whose priorities are my grown children and their partners, my 3-year-old grandson, 20-month-old granddaughter, and my longtime partner. I have a huge passion for all animals, and love doing Pilates, aerobics, Zumba Gold, and weight training. I became a pescetarian about 10 years ago. I was diagnosed with a very specific high-risk subtype of Chronic Lymphocytic Leukemia (CLL) in April 2019, three weeks after the tragic passing of my third child, my 26-year-old daughter. I was so devastated by her death, and now facing an unknown diagnosis, I turned to the Leukemia &amp; Lymphoma Society (LLS) for support. Fast-forwarding to now, I have been on the “Light The Night” Executive Leadership Committee for the Westchester/Fairfield LLS for four years, and my team “Lynne’s Hope” has raised almost $200,000 for the cause, for which I am extremely proud. I am blessed to be under the care of a world-renowned CLL specialist. I want cancer survivors to know there is hope for us to live a long life because of all the constant new medical treatments that are being discovered every day. I have been in treatment for two years for CLL and am doing great! My mantra is, “I will not let Leukemia define me; I will define it!”</p>

Schizophrenia is a persistent and often disabling mental illness that affects how a person thinks, feels, and behaves. In 2020, there were nearly 2.8 million people in the U.S. impacted by schizophrenia, and it is one of the top causes of disability worldwide.<u></u><u></u></p>
<p>Schizophrenia is not a one-size-fits-all condition, and people often find themselves in a cycle of discontinuing and switching therapies. Studies have shown that approximately 40% of people with schizophrenia do not respond to therapy, and up to 60% experience a partial or inadequate improvement or intolerable side effects during therapy.<u></u><u></u></p>
<p>The FDA recently approved a new medication for the treatment of schizophrenia in adults, representing the first new class of medicine in more than 30 years. <p>Dr. Gus Alva is the medical director of ATP clinical research (a premiere private clinical research company specializing in neuropsychiatric investigations). He is an Associate Professor in the Department of Psychiatry and Human Behavior at the University of California, Irvine (UCI). He is a forensic psychiatric consultant for the Orange County Superior Court and a diplomate of the American Board of Psychiatry and Neurology.</p>
<p>Dr. Alva received his medical degree from the Autonomous University of Guadalajara (UAG). Residency training was completed at the University of California, Irvine Medical Center in the Department of Psychiatry and Human Behavior, where he served as chief resident during his final year of residency.</p>
<p>Dr. Alva has served as a principal investigator for the past 10 years conducting investigations in a myriad of neuropsychiatric conditions: Alzheimer&#8217;s and other dementias, depression, anxiety disorders, bipolar disorder and psychotic conditions. As a clinician and investigator, Dr Alva&#8217;s focus is on the genetics, neuroimaging, and latest treatments available for each aforementioned condition, as well as aspects of ethnicity and their correlation within clinical situations. He has published numerous articles, and presents at both national and international meetings and conferences.</p>
<p>He is the immediate past-president of the Orange County Psychiatric Society. A new survey sheds light on the many ways that acute pain can disrupt daily life. <u></u><u></u></p>
<p>Although pain is one of the most common reasons that people visit the doctor, managing it can be complicated.</p>
<p>So, what’s a patient—or a doctor – to do?  Here to talk about the unmet need in treating acute pain is Dr. David Dalury, Professor of Orthopedic Surgery (Emeritus) at the University of Maryland.</p>

<p>The results show how patients’ lives are affected by acute pain:</p>
<p>70% say it limits them from walking and exercising.<br />
69% says it limits their sleep.<br />
65% says it affects their hobbies and leisure activities.<br />
65% say it made them feel irritable or emotionally drained.<br />
Patients reported missing an average of 19 working days annually.</p>
<p>Pain is one of the most common reasons that people visit the doctor; however, treatment options for the various types and causes of pain remain limited.i,ii,iii</p>
<p>Approximately 40 million Americans with acute pain are prescribed an opioid to manage their discomfort each year.vii Although opioids can be effective, they also carry a labeled risk of addiction.viii In fact, roughly 85,000 U.S. adults are expected to develop opioid use disorder (OUD) within a year of their prescribed treatment of acute pain.vii</p>
<p>Health care providers may prescribe either acetaminophen or nonsteroidal anti-inflammatory drugs (NSAIDs) as alternatives for milder pain or additions to opioids for severe acute pain; while these treatments are generally well tolerated, they may have limited effectiveness, particularly for moderate-to-severe acute pain.viii,ix</p>
<p>The survey also shows that:</p>
<p>77% of patients say they are interested in trying a different pain medication if they experience acute pain again.<br />
52% say they are seeking a new pain medication with fewer side effects than their last medication.<br />
31% say they stopped taking their medication before their pain was resolved.<br />
On Wednesday, December 4th, Dr. David Dalury Professor of Orthopedic Surgery (Emeritus) at the University of Maryland will be available to discuss the survey results and help raise awareness about the unmet need in current pain management options and offer solutions.</p>

<p>Restless Legs Syndrome (RLS) is a disruptive sleep-related movement disorder characterized by a strong urge to move one or both legs (and sometimes the arms), usually accompanied by an uncomfortable feeling in the legs.  <b><a href="https://www.google.com/url?q=http://rls.org/&amp;sa=D&amp;source=calendar&amp;usd=2&amp;usg=AOvVaw36Sx1Df4BKR0Q0NO7kDC7t" target="_blank" rel="noopener">rls.org</a></b></p>
<p>The condition is common and impacts 2-3 % of adults in the United States.</p>
<p>RLS is most prominent in the evening or at night and symptoms may get better with movement. This often leads to sleep disruptions that impact quality of life and can lead to difficulty falling asleep or staying asleep, the urge to get out of bed to stretch or move their legs, fatigue or daytime sleepiness, behavior or mood changes, difficulty paying attention, remembering things or contextualizing as well as depression or anxiety.</p>
<p>Now, for the first time in 12 years, the American Academy of Sleep Medicine (AASM) has published an update to its Clinical Practice Guidelines for the treatment of RLS. These guidelines may have a major impact on how people with RLS are treated with the goal of finding the most effective treatment for symptom relief.</p>
<p>Dr. John Winkelman, MD, PhD, Chief of the Sleep Disorders Clinical Research Program in the Department of Psychiatry at Massachusetts General Hospital will be available to discuss the latest advances in RLS treatment guidelines and what patients and physicians should be aware of.</p>

<p>Researchers have discovered how a cell surface protein called Aplp1 can play a role in spreading material responsible for <a class="lar_link lar_link_outgoing" href="https://www.sciencealert.com/go/IYl" target="_self" rel="nofollow noopener" data-linkid="73029" data-postid="130148">Parkinson&#8217;s</a> disease from cell-to-cell in the brain.</p>
<p>Promisingly, an FDA-approved <a class="lar_link lar_link_outgoing" href="https://www.sciencealert.com/cancer" target="_self" rel="nofollow noopener" data-linkid="73077" data-postid="130148">cancer</a> drug that targets another protein called <a href="https://en.wikipedia.org/wiki/Lymphocyte-activation_gene_3">Lag3</a> – which interacts with <a href="https://en.wikipedia.org/wiki/APLP1">Aplp1</a> – blocks the spread in mice, suggesting a potential therapy may already exist.</p>
<p>In a <a href="https://link.springer.com/article/10.1038/s41467-024-49016-3">new paper</a>, an international team of scientists describes how the two proteins work together to help harmful <a href="https://en.wikipedia.org/wiki/Alpha-synuclein">alpha-synuclein</a> protein clumps get into brain cells.</p>
<p>&#8220;Now that we know how Aplp1 and Lag3 interact, we have a new way of understanding how alpha-synuclein contributes to the disease progression of Parkinson&#8217;s disease,&#8221; <a href="https://www.hopkinsmedicine.org/news/newsroom/news-releases/2024/06/new-study-suggests-cancer-drug-could-be-used-to-target-protein-connection-that-spurs-parkinsons-disease">says</a> Xiaobo Mao, a neuroscientist from Johns Hopkins University in the US.</p>
<p>&#8220;Our findings also suggest that targeting this interaction with drugs could significantly slow the progression of Parkinson&#8217;s disease and other neurodegenerative diseases.&#8221;</p>
<p><a href="https://www.who.int/news-room/fact-sheets/detail/parkinson-disease">More than 8.5 million people</a> globally have Parkinson&#8217;s, the <a href="https://www.ninds.nih.gov/current-research/focus-disorders/parkinsons-disease-research/parkinsons-disease-challenges-progress-and-promise">second most common neurodegenerative disease</a> after <a class="lar_link lar_link_outgoing" href="https://www.sciencealert.com/go/IaO" target="_self" rel="nofollow noopener" data-linkid="73015" data-postid="130148">Alzheimer&#8217;s</a>.</p>
<p>As a progressive movement disorder, it&#8217;s usually only diagnosed when symptoms show, which include tremors, stiffness, balance problems, speech difficulties, disturbed sleep patterns, and mental health issues. Currently incurable, the disease means patients may eventually struggle to walk or speak.</p>
<p>Parkinson&#8217;s symptoms mainly result from the death or impairment of dopamine-producing neurons in the brain&#8217;s <a href="https://en.wikipedia.org/wiki/Substantia_nigra">substantia nigra</a>, a region involved in fine motor control. This is thought to be caused by <a href="https://en.wikipedia.org/wiki/Lewy_body">Lewy bodies</a>, which are abnormal clumps of protein mostly consisting of misfolded alpha-synuclein that travel between neurons.</p>
<p>Alpha-synuclein typically maintains functional communication between neurons, but problems arise when it becomes misfolded and insoluble. That said, identifying whether this is <a href="https://www.sciencealert.com/a-confused-immune-system-could-be-behind-parkinson-s-disease">a cause of Parkinson&#8217;s or a symptom</a> is difficult.</p>
<p><a href="https://www.science.org/doi/full/10.1126/science.aah3374">Past studies</a> on mice <a href="https://doi.org/10.1073/pnas.2011196118">found Lag3 binds to alpha-synuclein proteins</a> and spreads Parkinson&#8217;s disease pathology in neurons. While deleting Lag3 significantly impedes this process, it does not completely prevent it, indicating another protein was also implicated in neurons taking in misfolded alpha-synuclein.</p>
<p>&#8220;Our work previously demonstrated that Lag3 wasn&#8217;t the only cell surface protein that helped neurons absorb alpha-synuclein, so we turned to Aplp1 in our most recent experiments,&#8221; <a href="https://www.hopkinsmedicine.org/news/newsroom/news-releases/2024/06/new-study-suggests-cancer-drug-could-be-used-to-target-protein-connection-that-spurs-parkinsons-disease">says</a> Johns Hopkins neuroscientist Valina Dawson.</p>

<p>The scientists conducted tests with genetically modified mice that were missing either Aplp1 or Lag3, or both. They found Aplp1 and Lag3 can each independently help brain cells absorb harmful alpha-synuclein, but together they significantly increase the uptake.</p>

<p>When mice were missing both Aplp1 and Lag3, 90 percent less of the harmful alpha-synuclein entered healthy brain cells, meaning a greater amount of the harmful protein clumps was blocked with both proteins missing compared with a deletion of just one.</p>
<p>The researchers gave normal mice the drug <a href="https://en.wikipedia.org/wiki/Nivolumab/relatlimab">nivolumab/relatlimab</a>, a melanoma medication that contains a Lag3 <a class="lar_link lar_link_outgoing" href="https://www.sciencealert.com/antibody" target="_self" rel="nofollow noopener" data-linkid="73081" data-postid="130148">antibody</a>, and found that it also stopped Aplp1 and Lag3 from interacting, again almost completely blocking the formation of disease-causing alpha-synuclein clumps in neurons.</p>
<p>&#8220;The anti-Lag3 antibody was successful in preventing further spread of alpha-synuclein seeds in the mouse models and exhibited better efficacy than Lag3-depletion because of Aplp1&#8217;s close association with Lag3,&#8221; <a href="https://www.hopkinsmedicine.org/news/newsroom/news-releases/2024/06/new-study-suggests-cancer-drug-could-be-used-to-target-protein-connection-that-spurs-parkinsons-disease">says</a> Ted Dawson, a neuroscientist at Johns Hopkins University.</p>
<p>The next step will be to test the Lag3 antibody on mouse models of Parkinson&#8217;s disease and Alzheimer&#8217;s – where <a href="https://doi.org/10.1002/advs.202303775">research has pointed to Lag3</a> as a target too.</p>

The FDA Recently Approved a New Oral Treatment</p>
<p>There is now a new treatment option for the estimated 2 million people in the United States living with moderate-to-severe plaque psoriasis1,2,3 whose symptoms may make day-to-day life more challenging. For those that aren’t familiar with psoriasis, it is a widely prevalent, chronic, systemic immune-mediated disease.4</p>
<p>And up to 90% of patients with psoriasis have psoriasis vulgaris, also known as plaque psoriasis,5 which is characterized by distinct round or oval plaques typically covered by silvery-white scales.</p>
<p>One of the challenges that both doctors and patients face is that despite the availability of therapies, many people living with plaque psoriasis in the United States are untreated or undertreated.5,6</p>
<p>But good news has arrived. The U.S. Food and Drug Administration (FDA) has officially approved a new treatment for adults with moderate-to-severe plaque psoriasis who are candidates for systemic therapy or phototherapy.  </p>
<p>Dr. April Armstrong, dermatologist and clinical investigator in one of the treatment’s trials and Associate Dean and Professor of Dermatology at the University of Southern California, alongside Leah M. Howard, J.D., President and CEO of the National Psoriasis Foundation, will discuss the impact of psoriasis and a new, FDA-approved oral treatment for moderate-to-severe plaque psoriasis. *</p>
